How much to tip a chartered cab?

I have called and requested that a cab take me from one small town in virginia to another town. The cab company has a fixed fee of 45 dollars between these two towns. Am I expected to tip on top of this? As a New Yorker, I'm not sure what rules apply down here, and with fixed rates.
